Terms and Conditions

Privacy: I understand that the information contained in the OSF MyChart account is not my legal medical record or the HIPAA designated record set and that I may request and obtain a copy of my medical record from the applicable healthcare provider.

I understand that the privacy of my information is very important to my provider and that I may review my provider's Notice of Privacy Practices (NPP) for a thorough discussion of how they may gather, use and protect confidential information. A link to each provider's NPP can be found below:

Electronic Messaging: I understand that OSF MyChart should never be used for urgent health care matters. The routine turnaround time for a response to electronic messages is 2-3 business days. Therefore, for all urgent matters, I should dial 911, or go to the nearest hospital emergency room. I understand that entries I make into the OSF MyChart record may become part of my medical record.

OSF MyChart Username and Password: I understand that when I initially enrolled in OSF MyChart, I was provided with an activation code. The activation code I received gave me the opportunity to create a personal login name (called an "OSF MyChart Username") and a password. Once the activation code has been used, it will no longer be valid. I understand that it is extremely important that I keep my login name and password completely confidential. Anyone with access to my login name and password will be able to access my confidential medical information. If I have signed up as the proxy on another person's record, the inappropriate user will be able to view all accounts where I am the proxy, including my child's record. That person will also be able to read my messages and send new messages to my doctor as if they were me. I understand and agree that it is my responsibility to select a confidential password, to maintain my password in a secure manner, and to change my password if I believe that it may have been compromised in any way. I may change my password at any time.

E-Mail Privacy: I am aware that I will receive Internet E-Mail messages notifying me of new information that has been posted to my OSF MyChart account. These E-Mails will not contain any confidential medical information.

Security and Confidentiality: I understand that OSF is committed to maintaining the security of my information contained in my OSF MyChart account. OSF utilizes the industry standard security protections available at the time of install. I understand that my activities in OSF MyChart may be tracked by computer audit.

Information Does Not Constitute Medical Advice: OSF Healthcare System assumes no responsibility for how I use the information that I obtain from the OSF MyChart site. I should consult a physician or other health care provider regarding my condition and how the OSF MyChart content may or may not apply to me.

Termination of Access: OSF Healthcare System, and/or my physician may restrict or terminate my access to OSF MyChart at any time, and at their sole discretion whether or not the restriction is based on my violation of these Terms and Conditions. Access to OSF MyChart is a license and a privilege granted by OSF Healthcare System and does not confer any rights to the user. If access is restricted for planned maintenance of the OSF MyChart system, reasonable effort will be made to communicate the downtime to all users in advance.

Restrictions on Use and Ownership: All of the pages in the OSF MyChart site and any material made available for download are the property of OSF Healthcare System and/its subsidiaries and affiliates. The content on the OSF MyChart site is provided for the OSF Healthcare System and patients only. The OSF Healthcare System logo, trademarks, service marks, and logos used on the OSF MyChart site, including OSF MyChart and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on the OSF MyChart site should be construed as granting any license or the right to use any trademark, service mark, or logo displayed on the OSF MyChart site without the express permission of OSF or the third party that may own the trademark, service mark or logo.

Accuracy and Integrity of Information: OSF Healthcare System attempts to ensure the integrity of the data on the OSF MyChart site, however it makes no guarantees whatsoever as to the correctness or accuracy of the information on the Site. It is possible that the Site could include typographical errors, inaccuracies, or other errors. In the event that an inaccuracy arises, I will inform OSF Healthcare System so that it can be corrected. Information contained on the Site may be changed or updated without notice.

No Warranties: Any and all warranties of merchantability and fitness for a particular purpose are expressly disclaimed. Access to and use of OSF MyChart is provided "as is," and OSF Healthcare System makes no representations or warranties of any kind as to its function, suitability, or accuracy.

Limitation of Damages: As a condition of use of OSF MyChart, I agree that the limit of liability of OSF Healthcare System, its directors, officers, employees, and agents, for any breach of an obligation in respect to OSF MyChart shall not exceed the sum of One Hundred Dollars ($100.00).

OSF MyChart Text Messaging (SMS) Program

Description: OSF MyChart Text Messaging allows mobile subscribers to receive notifications of new messages and test results from OSF MyChart through SMS (text) messages straight to the subscriber's phone.

Supported Carriers: Alltel, Appalachian Wireless, AT&T, Bluegrass Cellular, Boost Mobile, Cellcom, Cellular South, Centennial Wireless, Cincinnati Bell, GCI, Immix Wireless, Inland Cellular, IV Cellular, Nex-Tech Wireless, nTelos, Sprint PCS, T-Mobile, U.S. Cellular, United Wireless, Verizon Wireless, Virgin Mobile USA, and West Central Wireless.

Cost: There are no premium charges for choosing to receive OSF MyChart text message notifications. However, Message and Data rates may apply.

Message Frequency: If you select to receive notifications from OSF MyChart, you will receive one message each time you have a new message to your OSF MyChart account. You will also receive one message to notify you of test results that are viewable from your OSF MyChart account. You can manage your notification preferences by clicking on Notifications from the Profile menu.

Opt-out of our Service: If you choose to no longer receive text messages from OSF MyChart, text STOP at any time to 878787. An unsubscribe message will be sent to your mobile number confirming the cancellation, but no more messages will be sent from this program after that one.

Privacy Policy: OSF MyChart's top priority is the privacy of our users. The following is provided to address any concerns you may have:

- OSF MyChart will never, under any circumstances, sell or distribute your cell phone number to third parties or OSF MyChart clients for whom you have not approved.
- OSF MyChart will never directly market to you any services for which you have not opted in, either by cell phone, text message or email.
- OSF MyChart will never distribute any personal information about you, including your phone number, name, billing information or any other piece of identifying information.

Happy Together in OSF MyChart

You can use OSF MyChart to view your health information from other organizations where you and your family receive care through a new feature called Happy Together. That means if you receive care at different organizations, you no longer have to log into multiple MyChart accounts, as long as those organizations have also implemented Happy Together. This feature makes managing your healthcare needs from OSF MyChart even more convenient!

It is your choice whether or not to link your healthcare information from other organizations. By doing so, you consent to viewing the aggregated information from these other organizations.
